Hyaluronic Acid — The Hydration Matrix Molecule

Hyaluronic acid (HA) is a glycosaminoglycan that forms the hydration matrix of connective tissue, skin, and synovial fluid. A single HA molecule can bind up to 1,000× its weight in water. HA content in skin and joints declines dramatically with age, driving skin aging, joint degradation, and reduced lubrication. Oral HA supplementation has demonstrated both skin and joint benefits in RCTs.

Mechanism of Action

Oral HA is partially absorbed as oligosaccharides that stimulate fibroblasts to produce endogenous HA via CD44 receptor signalling. In joints, HA provides viscosity and lubrication for cartilage. In skin, it maintains the extracellular matrix hydration and signals keratinocytes to produce more HA. Low molecular weight HA (<50 kDa) may have superior oral bioavailability.

Human Trial Evidence

A 2017 Nutrition Journal RCT showed 120 mg/day oral HA significantly improved skin moisture and elasticity over 12 weeks. A 2012 Nutrition Journal RCT showed 80 mg/day HA reduced knee pain and improved function in patients with knee osteoarthritis over 12 months. Multiple Japanese trials confirm skin hydration benefits.

Dosing Protocol

80–200 mg/day for joint and skin benefits. Low molecular weight HA (50–300 kDa) is most studied for oral use. Best taken with food. Effects on skin visible after 4–8 weeks; joint benefits after 8–12 weeks. Topical HA is complementary but acts differently (surface hydration only).

Safety & Contraindications

Excellent safety profile. GRAS status. No significant adverse effects in trials. Derived from fermentation (vegan) or rooster comb (animal). Theoretical concern in cancer (HA promotes cell migration) — not confirmed clinically at supplement doses. No drug interactions identified.
